*If your student has an allergy, please complete this form and send it to the Food Services Office or email it to foodservicessec@mdusd.org.
*If your student has dietary (ie., vegetarian, pescatarian, etc.) or religious restrictions please email foodservicessec@mdusd.org. Include your name and relationship to student, student's school, student's name, teacher, and requested accommodation.
Income Eligibility Guidelines
The California Universal Meal program continues in the 2024-2025 school year. All students may collect one breakfast and one lunch during the school day at no cost. Your enrolled student is already set up to receive these meals. No action is required.
Every school in MDUSD now qualifies for Community Eligibility Provision (CEP). Food & Nutrition Services is accepting Household Income Forms from all MDUSD households. The Household Income Form can be filled out in HomeLink or contact your school office for a form.
Though Free and Reduced Price Meal Applications are prohibited, households who would qualify for the Federal program continue to be captured via Household Income Form and any additional benefits (Summer EBT cards, no-cost or discounted SAT, ACT, AP testing fees, discounted utilities and cable, etc.) still apply. It is of the utmost importance that a Household Income Form is collected from every household in MDUSD to ensure funding is allocated appropriately into your school.
